CLAIMS PROCEDURE FOR CLUB OFFICIALS AND CLAIMANTS

CLUB / ASSOCIATION OFFICIAL

Ensure the injury has been recorded on the score/team sheet and give a copy of this to the claimant to send in with the claim form.

Advise the claimant to notify Sportscover Australia within 30 days of the injury; either by phone or via our email address and a claim form will be sent to them.

Please note: Should claimants have any questions regarding the policy and its benefits please encourage them to contact Sportscover Australia immediately. Trained staff, are on hand to answer any questions they may have.

CLAIMANT

Notify Sportscover Australia of your injury within 30 days and a claim form shall be sent to you.

Return the completed claim forms to Sportscover Australia within 90 days from the date stamped on the claim form. (This is regardless of whether treatment is completed or not)

Obtain a copy of the score sheet from the game in which you were injured.

In order for your claim to be assessed efficiently, please send to Sportscover Australia at your earliest convenience the completed Physicians statement, original non medicare medical receipts and statements from your private health insurer, and if claiming for loss of income please include the Details of Employment statement.

Please notify Sportscover Australia in writing when all treatment is complete to be sure of an early settlement.

ONLY ORIGINAL CLAIM FORMS WILL BE ACCEPTED
